Nurse (RN) (Contract) (Ref.?SSPDHC/255326)

Requirements:

• Certificate of Registration (Part I or Part II) with the Nursing Council of Hong Kong and with valid Registered Nurse Practicing Certificate

• Completion of the Post-registration Certificate Course in Gerontology; or be a Fellow member of The Hong Kong College of Gerontology Nursing; or be recognized under the Voluntary Scheme on Advanced and Specialized Nursing Practice in Gerontology Nursing; or possess a recognized postgraduate certificate or diploma in Gerontology Nursing is preferred and will be given priority

• Minimum of five years of post-registration experience in clinical experience; and experience in geriatrics, long-term care, or community health is highly preferred

• Mature, detail-oriented, able to work independently and collaboratively within a multidisciplinary team and demonstrates a systematic approach to clinical work

Job Duties:

• Provide screening, risk-based health assessment, personalized care coordination and interventions for members aged 65 or above, covering seven key domains: mobility, cognitive, visual , nutrition, psychological capacity, hearing and urinary conditions

• Serve as case manager for members aged 65 or above with specific elderly conditions, facilitating integration into the Primary Co care Network or arranging priority referrals to HA Family Medicine Clinics

• Work closely with different stakeholders and leveraging medical-social collaboration to provide health assessments at District Elderly Community Centre (DECC) and Neighbourhood Elderly Centre (NEC) for their eligible members

• Engage and collaborate with Family Doctors to provide follow up services for elderly individuals requiring referrals after screening and assessment across the seven domains

• Supervise Care Assistants and conduct regular in-service training on falls prevention, dementia care, wound management, and basic life support

• Participate in training programmes, simulation drills, clinical audits, and quality assurance activities

• Undertake other duties as assigned by the supervisor.
