Job Purpose
The Clinical Patient Navigator is responsible for guiding patients through their healthcare journey by ensuring smooth coordination of clinical services, facilitating communication between patients and healthcare providers, and supporting high-quality, patient-centered care. This role acts as a key liaison between patients, families, and multidisciplinary medical teams to optimize clinical outcomes and patient experience.
Key Responsibilities
Serve as the primary clinical liaison for patients throughout their care journey.
Coordinate patient care plans with physicians, nurses, and other healthcare professionals.
Ensure timely access to appropriate medical services, consultations, and follow-ups.
Assist patients and families in understanding diagnoses, treatment options, and care plans in a clear and compassionate manner.
Monitor patient progress and ensure continuity of care across departments and services.
Support discharge planning and post-treatment coordination, including referrals if needed.
Facilitate communication between patients (including international patients) and medical teams.
Identify and resolve barriers that may affect patient care or service delivery.
Maintain accurate documentation in Electronic Medical Records (EMR) systems.
Collaborate with case management, insurance, and administrative teams when required.
Ensure compliance with hospital policies, clinical guidelines, and patient safety standards.
Contribute to improving patient experience and service quality initiatives.
Requirements
Medical Doctor (MD) degree from an accredited university.
Valid and active Surat Tanda Registrasi (STR) and eligible to obtain Surat Izin Praktik (SIP) in accordance with applicable regulations.
Minimum 2–3 years of clinical experience in a hospital or healthcare setting.
Strong understanding of clinical workflows, patient journey, and multidisciplinary care coordination.
Ability to effectively communicate medical information to patients and families in a clear and compassionate manner.
Excellent communication, interpersonal, and patient counseling skills.
Proven ability to coordinate with physicians, nurses, and other healthcare professionals to ensure seamless patient care.
Strong commitment to patient-centered care, safety, and service excellence.
Good organizational, analytical, and problem-solving skills.
Proficient in Electronic Medical Records (EMR) systems and Microsoft Office applications.
Fluent in Bahasa Indonesia and English (written and spoken).
Experience in handling international patients or working in an international hospital setting is an advantage.
Able to work effectively in a fast-paced, multidisciplinary environment.
